Now doctors say a bystander can recognize a stroke by asking three simple questions:

S * Ask the individual to SMILE.
T * Ask the person to TALK and SPEAK A SIMPLE SENTENCE.
(Coherently) i.e. It is sunny out today)
R * Ask them to RAISE BOTH ARMS.

If he or she has trouble with ANY ONE of these tasks, call 911 immediately and describe the symptoms to the dispatcher.

Informed Consent Form from Life Chiropractic College (pdf) - although I am glad to see an informed consent form about vertebral artery dissections, I believe their estimated number of vertebral artery dissections caused by chiropractic adjustments is very low at between 1:2 Million and 1:1 Million.

I also believe these consent forms should be required by law in all chiropractic offices.
